How do I learn more about supportive care and costs with Live Long Well Care?

Simply call the Client Services Manager at your selected location (info below). They will coordinate a Consultation and recommend Care Options best suited to your needs.

  1. Consultation: At no cost, a professional consultant will come and speak with you or a family member to determine your needs and answer your questions. We will recommend services based on findings of the visit and short questionnaire.
  2. Care Options: You will be offered supportive care services by highly trained, licensed, and certified caregivers. These services are provided to best suit your needs at an hourly rate or as convenient monthly fees. Examples of our most popular services are listed on the opposite side of this sheet.

Popular Care Options and Fees

Supportive Care Services

Supportive Care Services encompass any and all services offered by caregivers at an hourly rate. Services include Personal Care, Nursing Care, Companion Care, Homemaker Services, Respite Care, Live In & Overnight Care. Supportive Care Services can be provided 24 hours a day, 7 days a week. A minimum of 30 minutes per visit are required. Holiday rates are 1.5 times the hourly rate. Visits requiring a Registered Nurse are billed at $65 per visit. Cancelation fees may apply.

Medication Management

For a low monthly rate, a licensed professional will set up medication organizers to ensure the accuracy of when and how often each medication is taken. Medication management also includes picking up and/or dropping off medications to be refilled from the local pharmacy of your choosing (mileage costs may apply). Printing a medication list for Physician appointments and having conversations with physicians and/or pharmacy. Additional needs may result in additional charges.

Monthly Supportive Care Packages

A consistent daily service. Dependent upon your needs, we offer three different packages to choose from. A trained caregiver will assist you with your daily needs. This can be used for a multitude of services. You will have the flexibility to have different needs supported each day. An example could be: On Monday, I want assistance with laundry, Tuesday, I want assistance with weekly meal preparation, but on Wednesday, I want my bed sheets changed and laundered.

  • Care Assist Level I –

    for up to 1 hour of service or 3 visits per day, not to exceed 1 hour. A trained caregiver will assist you with your daily needs.
  • Care Assist Level II –

    for up to 1.5 hour of service or 4 visits per day, not to exceed 1.5 hours. A trained caregiver will assist you with your daily needs.
  • Care Assist Level III –

    for up to 2 hours of service or 5-6 visits per day, not to exceed 2 hours. A trained caregiver will assist you with your daily needs.

WellerLife Start

A unique program between the hours of 6:00 am and 11:00 am to help your day get started. Our Weller Life Start program assists you with getting out of bed and getting the day started. It includes services such as transfer, dressing, personal hygiene assist, and continental breakfast set-up.

WellerLife Turn In

A unique program between the hours of 7:00 pm and 11:00 pm to help finish your day. Our Weller Life Turn In program assists you with getting ready and/or into bed for a restful night sleep. It includes services such as transfer, dressing, personal hygiene assist, and prepping a light midnight snack.

Supportive Connections

Live Long Well Care offers telephone calls tailored to you or loved ones needs. This service is a great alternative and supplement to home visits. Two of our most popular options include:

  • Phone Medication Reminders: A caregiver will call and stay on phone until the client states medications have been taken. An initial $25 administrative fee is required.
  • Phone Wellness Checks: A personalized phone call can be set up to check on you or your loved one. This can simply be a short discussion or personalized based on needs. We all need someone to talk to. An initial $50 administrative fee is required.
